TriageHFPlus app for iPhone and iPad

TriageHFPlus App is now available on IOS to allow patients to report health status to the hospital by answering simple questions: the answers will be available and consultable in the hospital.

It is an app dedicated to patients affected by heart failure included in the One Hospital ClinicalService project of their hospital.

Your doctor will tell you how and when to use this app and provide you with login credentials.
